Loan Cancellation Policy

Borrowers wishing to cancel their loan may request cancelation in writing at the below address:

The writing must include the billing account number, the amount requested to be canceled and a statement of why the borrower believes they are entitled to cancelation.  HFA reserves the right to decline a request for cancelation; submission of a cancelation in the form above does not guarantee HFA will cancel the loan. Approved cancelations may result in the patient owing their Provider of Medical Services, for which the loan was originally obtained, a service rendered balance.
